Service Agreement
By engaging Starlight Plan Management for NDIS plan management services, you agree to these terms and conditions.
Our Services
Starlight Plan Management provides:
- NDIS plan management and administration
- Provider payment processing
- Monthly budget statements
- Plan monitoring and reporting
- Support and guidance
Your Responsibilities
As a participant, you agree to:
- Provide accurate and up-to-date information
- Notify us of any changes to your plan or circumstances
- Use services in accordance with NDIS guidelines
- Maintain appropriate records
Fees and Payments
Our plan management services are provided at no cost to NDIS participants. Fees are paid directly by the NDIA as part of your plan management budget.
Limitation of Liability
Starlight Plan Management's liability is limited to the extent permitted by law. We are not responsible for decisions made by the NDIA or service providers.
Termination
Either party may terminate this agreement with appropriate notice. We will assist with the transition to another plan manager if requested.
Governing Law
These terms are governed by the laws of Victoria, Australia.
